Bundled Loop in Customer Service - Residential Telephone Service

In November 2010, I called Verizon to request a disconnect of service on December 31, 2010 of my residential telephone service. It was in making this request that I found out that Verizon had accidently rebundled my service in August 2010 after I had already met the bundling contract obligation. I was told it was a mistake, and that a supervisor would correct it in their system. Four supervisors tried to cancel the early bundling termination fee that would show up on my final bill, however, I was told that the COES (may be misspelled) system was down and that when it came up, the fee would be removed. I was given a confirmation number. I also have the name and location of the person I talked to. I received a final bill which I paid and was told and received a refund from Verizon. It was after I received my refund that I also got a Final bill with a new account number billing me for the termination fee. I called, and was told it was an error and would be corrected since this bill was for service from 12/31/10 to 1/18/2011 (long after service was off and I had moved.) I have names and confirmation numbers from that day as well. I was told that I had to do nothing and that all charges had been cleared. Recently, this week, March 1st, I received a letter from a debt collection company where Verizon had reported the error they made to the agency. I called both the agency and Verizon and was transferred from Billing to Collections, to Credit Reporting, then back to Billing. The cycle is never-ending, I have proof of the error made and have encountered rude sales persons with Verizon who have been rude, one person even told me to shut up and listen, and even hung up on me. I cannot get this resolved and feel that I have been bundled into their system. This is so unfair and my credit is at risk of being ruined over their mistake. Can someone please help me get through, Verizon's supervisor told me yesterday that he cannot talk to me without my going through the debt collector and asking them to send Verizon bck the bill? With that I am appalled, please help.
